Please arrive 5 minutes early to check in and register.

Please call/email if unable to make an appointment.

You have "missed" your appointment if:

  • 5 minutes late for a 15 minute appointment or
  • 10 minutes late for a 30 minute appointment.

    15 minutes late for any appointment is considered a "no show."

    Cancellations/No Shows/Fees:

  • no fee if 2 days or greater notice given
  • 20$ fee if cancellation made the day before a visit
  • 40$ fee for "missed appointments", "No Show" or same day cancellations.

    • There is no fee, if you accept a same-day open time slot.

  • 3 No Shows - dismissal from the practice

    Insurance

    I am accepting the following insurances:

  • Medicare, Aetna, BCBC, EHP, Cigna, Coventry, United
  • Insurance claims are submitted as a courtesy.

    • Don't see your carrier? Call or email us.

    Copays are due in full at the time of visit.

    Fee Schedule

    For self-pay/cash paying patients,

    Below are approximate fee ranges for common services:

  • Pre-operative eval $145 - $270
  • Physical - established $145 - $175
  • Physical - new patient $175 - $220
  • Routine follow-up $60 - $130
  • Sick Visit - established $60 - $130
  • Sick Visit - new patient $105- $215
  • Vaccinations $30 for administration plus the cost of the vaccine.
  • Forms (FMLA, etc. any form requiring review and assessment) $25
  • Medical Records - $0.70 per page plus postage and handling.

    Payment

    We accept Visa, Mastercard, personal checks, and cash.

    Account balances must be paid in full at time of appointment.

    New visitors are requested to pay with major credit card or cash.

    You are responsible for co-pays, adjustments and any non-covered services
